Unrestricted bequest language:
I hereby give and bequeath (________ dollars / __________ percent or all of the residue of my estate) (or, in the case of property, describe) to Beth Israel Deaconess Hospital–Plymouth ("BID Plymouth"), a Massachusetts charitable corporation, with an address of 275 Sandwich Street, Plymouth, Massachusetts, for the benefit of BID Plymouth to be used for its general charitable purposes.

Restricted bequest language:
I hereby give and bequeath [________ dollars / __________ percent or all of the residue of my estate (or, in the case of property, describe)] to Beth Israel Deaconess Hospital–Plymouth ("BID Plymouth"), a Massachusetts charitable corporation, with an address of 275 Sandwich Street, Plymouth, Massachusetts, for the benefit of BID Plymouth to be used for [specific purpose, program, department]. If at any time in the judgement of BID Plymouth it is impossible or impracticable to carry out the designated purpose, BID Plymouth shall determine an alternative purpose as near as possible to the donor’s original intent.
